Introduction
In certain industries, there are rules written in blood. Aviation and process industries are among them. A rule written in blood specifies the mandatory regulations that must be implemented in future productions or systems based on lessons learned from a painful experience. For example, in the fuel sector, the overflow of gasoline in a terminal resulting in a catastrophic incident led to the establishment of standards for tank overflow prevention systems and the improvement of safety standards. Another example is the Seveso Directive, which was decided to be implemented following the Seveso accident.
